All CUMC Students (full- & part-time)
For all students enrolled in higher education, including part-time, NY State requires proof of
immunity against the following:
- Measles: This can be met either by a positive measles titer indicating immunity, or by two
doses of measles or MMR vaccine. The fi rst dose must be after one year of age; the second after 15
months of age and at least 30 days after the first dose.
- Mumps: Positive titer or one dose MMR after one year of age
- Rubella: Positive titer or one dose MMR after one year of age
- Meningococcal Meningitis Response Form: State requires completion of the Meningococcal
Meningitis Response Form. This is done
online at the time
of registration. Receipt of the vaccine is optional. Information on the vaccine is available at
www.cdc.gov.
Full-time Students in Public Health, Nutrition, and the Graduate School of Arts & Sciences
Note: These requirements also apply to part-time students in Public Health and Nutrition who choose to
enroll in the Student Health Service or Insurance program.
- History & Physical: Within 12 months of entry Measles, Mumps & Rubella: Immunity as
described above
- Tetanus (Td) Immunization: Within 10 years of entry
- PPD: Within 3 months of entry (unless previously positive).
Note: Receipt of BCG is not a contraindication to performing a PPD.
- Chest X-ray: Within 6 months of entry (only if PPD is positive)
- Meningococcal Meningitis Response Form: This is done
online at the time
of registration. Receipt of the vaccine is optional. Information on the vaccine is available at
www.cdc.gov.
All Medical, Dental, Physical Therapy, and Full-time Nursing & Occupational Therapy Students
Note: These requirements also apply to part-time students in Nursing & Occupational Therapy who
choose to enroll in the SHS or Insurance program.
- History & Physical: Within 12 months of entry
- Measles: Titer showing immunity
- Mumps: Titer showing immunity
- Rubella: Titer showing immunity
An additional MMR immunization is required if any of the above titers are negative, inconclusive, or
equivocal
- Varicella: Titer showing immunity or 2 doses of Varicella vaccine at least 30 days apart
- Hepatitis B Immunization: 3 immunizations AND post-immunization titer showing immunity
- Hepatitis B Surface Antigen and Hepatitis B Antigen: Required if the post-immunization titer is negative
- PPD:Within 3 months of entry (unless previously positive).
Note: Receipt of BCG is not a contraindication to performing a PPD.
