Instructions prior to applying:

Please be mindful that each program has a different deadline. Visit our website for a list of deadlines.

  1. Request all official transcripts from post-secondary schools (sent to you directly)
  2. Request 3 recommendations (Please notify your recommendation providers that the recommendation can be completed online. If they prefer to submit it offline, please access the Recommendation Form here.)
  3. If you have not taken the GREs within the last 5 years, please schedule a test date at Our school code is: 2142. You will need to indicate the school code at the time of taking the exam. Applicants are recommended to take the GREs at least 2 weeks prior to the deadline. International applicants who studied outside of the U.S. are required to take the TOEFL exam. The school code is: 9828.
  4. You will need to work on your personal statement. Please keep in mind that each program may have a different essay question.
  5. Please update your resume or CV.
  6. Application fees are paid online once ready to submit an application. Payment must be provided with a debit or credit card ($65 for all programs). For fee waivers, please complete the Fee Waiver Request Form and submit it to the Office of Admissions at least 2 weeks prior to the deadline.

Application Instructions:

Applicants must submit an application online. As of August 15, 2013, the School of Nursing no longer accepts paper applications. Please be mindful that each program has a different deadline. Visit our website for a list of deadlines.

  1. Please make sure to complete all required fields on the application.
  2. All supplemental materials must be uploaded to the online application.
  3. Please indicate your GRE test scores (TOEFL scores, if applicable) on the online application. If you have not yet taken the GREs, please indicate your expected test date. It is strongly encouraged that applicants take the exam prior to the application deadline.
  4. Your personal statement should be no more than 2 pages, double-spaced and 12 point font.
  5. Upload your CV/resume.
  6. DNP (post-MS) applicants must also submit a case study.
  7. All applicants (except ETP) are required to upload a copy of state licenses and board certifications.

Transcript(s) Upload Instructions:

Transcripts should be uploaded on the Academic Section page of the online application. A transcript from each school must be uploaded as a separate document. Transcripts should not be combined into one document.

  • Ensure that applicant name, institutional name, terms, grades, and courses are not missed during the scanning process and that the scanned copy is clearly legible and can print on letter-size paper (8 ½" x 11").
  • Save your document as a PDF file.
  • Make certain that its size is less than 2000kb (2 MB). Scanning in grayscale or black and white may produce the best results.
  • Preview and ensure the transcripts are legible.
  • Attach your saved PDF transcript to the application.
  • You may also upload web-based transcripts/academic records. Please ensure that the applicant name, institutional name, terms, coursework, and grades are included.
  • Transcripts must be in English or translated into English. An official copy of the original transcript will be required if offered admission to the School of Nursing.

Applicants who submit transcripts and are offered admission will be required to provide official transcripts upon acceptance. If there are discrepancies between the self-reported academic work and official records, offers of admission will be rescinded.

Recommendation Instructions:

  • Each applicant is required to submit 3 recommendations that can attest to the applicant’s knowledge, skills, and potential as a student at the School of Nursing. Ph.D. students have specific recommendation requirements; please visit our website for further instructions.
  • You will need the following information from your recommendation provider to submit an online recommendation: name, email, telephone, organization, position title, and relationship to applicant.
  • Use your recommender's institutional or corporate email address. Submissions from anonymous email addresses (Gmail, Hotmail, Yahoo) may be subject to additional review.
  • Once you have provided the above information, an email will go out to your recommendation provider from the Office of Admissions.
  • Applicants will receive an email confirming once the recommendation has been submitted.
  • If your recommendation provider is submitting an offline recommendation, please provide them with the Recommendation Form and they should email it to
